Tera W. McIntosh, PharmD, BCACP

Dr. McIntosh completed her PharmD degree at the University of Kentucky College of Pharmacy. She completed a PGY1 Community Pharmacy Residency with UK COP and American Pharmacy Services Corporation.

Current practice areas include collaborative patient management at Bluegrass Community Health Center, a Federally Qualified Health Center and level 3 Patient Centered Medical Home in Lexington, Kentucky, and co-ownership of Capital Pharmacy, an independent pharmacy in Frankfort, Kentucky. Services for individuals with opioid use disorder with medications are offered at both practice sites.
