Biosketch
Dr. Azar serves as an Assistant Professor of Clinical Medicine at the Division of Hematology and Oncology at a School of Medicine as well as the Chief Quality Officer and Patient Safety at an urban Academic Health Center. He is board certified as an oncologist and hematologist. He served as the Medical Director of Quality Improvement and Patient Safety and the Medical Director for the inpatient Hematology/Oncology Unit at a Cancer Center from September 2010 until June 2014 and then served as the Medical Director of Quality and Patient Safety for the adult Academic Health Centers until 2018 when he accepted his current position. He is a founding member of the Center for Healthcare Innovation and Implementation Science since its inception in 2012. Their primary purpose is to discover and rapidly implement innovative healthcare solutions in the real world in order to transform the healthcare system into a highly reliable learning system that can accomplish the triple aim of “better health, better care at lower cost”. They have developed the novel change methodology of Agile Implementation and Innovation. He is also a founding member of the Certificate of Innovation and Implementation Science which aims to create a network of healthcare change agents by providing training and mentorship in Agile Implementation and Innovation.
