The University of Kentucky Division of Community Medicine of the Department of Family & Community Medicine, the Kentucky Ambulatory Network (KAN), Owensboro Health, and the Appalachian Osteopathic Postgraduate Training Institute Consortium (A-OPTIC) are excited to be partnering with CECentral to offer online community medical education to practicing clinicians.

Division of Community Medicine and the Kentucky Ambulatory Network (KAN):

The Division of Community Medicine is part of the Department of Family and Community Medicine at the University of Kentucky College of Medicine. Its programs and activities involve medical student and resident curricular design and development, research, and service activities. Housed within the Division of Community Medicine, the Kentucky Ambulatory Network (KAN) was established in 2000. Educators, researchers, and clinicians collaborate across the state of Kentucky and beyond to advance the field of Community Medicine and Population Health. The overarching goal of the Network is to improve the wellbeing of our patients and communities through meaningful primary care practiced based research. COURSE OVERVIEW:

These educational activities are designed for the practicing clinician; however, healthcare learners of any field may find this course as a strong supplement for their career goals.

These modules provide a basic knowledge about caring for populations and patients (not only individuals) necessary for the changing models of care and the way we deliver care. The purpose of these modules is to introduce the concepts and applications of Population Health Management and Quality Improvement. The population health management framework can be embedded into a primary care integrated system in a variety of ways. The processes and key components discussed in this module can be applied to your organization to assess your own capabilities and to guide the development of expanded and integrated care delivery models.
