Dr. Azar serves as an Assistant Professor of Clinical Medicine at the Division of Hematology and Oncology at a School of Medicine as well as the Chief Quality Officer and Patient Safety at an urban Academic Health Center. He is board certified as an oncologist and hematologist. He served as the Medical Director of Quality Improvement and Patient Safety and the Medical Director for the inpatient Hematology/Oncology Unit at a Cancer Center from September 2010 until June 2014 and then served as the Medical Director of Quality and Patient Safety for the adult Academic Health Centers until 2018 when he accepted his current position. He is a founding member of the Center for Healthcare Innovation and Implementation Science since its inception in 2012. Their primary purpose is to discover and rapidly implement innovative healthcare solutions in the real world in order to transform the healthcare system into a highly reliable learning system that can accomplish the triple aim of “better health, better care at lower cost”. They have developed the novel change methodology of Agile Implementation and Innovation. He is also a founding member of the Certificate of Innovation and Implementation Science which aims to create a network of healthcare change agents by providing training and mentorship in Agile Implementation and Innovation.

She holds her BA in Classical Studies from Centre College. Catherine currently works with treatment facilities to ensure they update essential information onto the website as needed. She also reaches out to new treatment centers in an attempt to onboard them to the website. She works with the FindHelpNowKY team in the process of managing interest from other states which may implement this website for their own communities. Since her interest lies in education, she is working on researching and making connections with Universities and Colleges throughout the state in an attempt to educate them on the website and its use.

Robin Huffman joined the Kentucky REC team as the Transformation Manager in November 2016. In this role, Robin oversees the Practice Transformation Network team and the Patient-Centered Medical Home/Specialty Practice team. Robin brings over 20 years of healthcare experience to our team, including work in health information management, process improvement, staff development and patient & family experience. Robin earned her Master’s degree in Healthcare Administration from the University of Kentucky and is currently pursuing additional education in Human Resources and Organizational Development with an emphasis in Leadership Development. Robin is certified in LEAN process improvement methodology and is a Six Sigma Green Belt.

Trudi Matthews, MA

Trudi manages all staff, programs, and operations of the Kentucky REC, a center charged with assisting health care providers, practices, and hospitals across Kentucky with using technology, connectivity, process improvement, and value-based care models to improve the quality and value of health care.

Trudi also serves as a policy advisor on the External Affairs team for UK HealthCare. In this role, she helps leadership remain informed about changing state and federal legislation, regulation, and policy related to quality, value, meaningful use of IT, and payment innovation.

Formerly, she served as Director of Policy and Business Development with the Health Collaborative, HealthBridge, and the Greater Cincinnati Health Council where she worked with one of the nation’s top regional health improvement and transformation teams.

She provided leadership and support for a wide range of technology and quality improvement initiatives including Cincinnati’s Beacon Community Program, the Tri-State Regional Extension Center, Aligning Forces for Quality, the Kentucky and Indiana State Health Information Exchange Programs, and the Nationwide Health Information Network, among other initiatives.

Trudi led grant and business development efforts which garnered more than $26 million in funding for regional health improvement programs.

She has served as Senior Policy Advisor with the Kentucky Cabinet for Health and Family Services as the coordinator for Kentucky’s statewide e-Health efforts. Trudi worked for the Council for State Governments where she managed CSG’s health policy activities for state legislators and executive branch officials.

Trudi earned a BA and MA in political science from Iowa State University.

Ann O'Hara, MPA

Ann O’Hara serves as the Outreach Coordinator for the Northern/Central part of the State with the KY Heath Information Exchange (KHIE), located within the Cabinet for Health and Family Services in Frankfort, KY. Building upon her extensive sales and marketing experience, Ann’s role includes client management serving as a multi-tiered liaison between healthcare organizations, our HITECH partners and the Exchange. Over the past eight years, Ann has assisted in developing and implementing efforts to target specific healthcare markets, providing resources and guidance on KHIE Services. Her role also includes community outreach and KHIE-specific event planning. Ann is passionate about improving healthcare for the citizens of KY.

Gary Ozanich, PhD

Gary Ozanich is Director of Northern Kentucky University’s Program in Health Informatics. He is the past National Co-Chair of the Interoperability and Health Information Exchange Workgroup for the Health Information Systems Society (HIMSS) and was past National Chair of the HIMSS HIE Committee. He has conducted extensive funded research and consulting projects on health information exchange structure, interoperability, services, and sustainability, as well as projects examining care coordination and transitions in care. His current projects include working on the interoperability strategy for the Commonwealth of Kentucky, applying novel clinical decision support solutions in the treatment of diabetes mellitus, and working for the Office of National Coordinator on the Electronic Health Record Reporting Project. He has a private sector background which includes more than 12 years as a Vice President on Wall Street where he was an analyst of the technology and telecommunications industries. Dr. Ozanich also worked as a consultant at Booz-Allen & Hamilton. Relative to other academic positions, he was Associate Director of the Institute of Tele-Information at Columbia University, and has been on the faculties of Michigan State University and University at Buffalo. He holds a Ph.D. from the University of Wisconsin-Madison.

Tierra Pinkins is the Program Director of the Great Lakes Practice Transformation Network, a 4-year $46 million program funded by the Centers for Medicare and Medicaid Services. She has an undergraduate degree in Health Services Management and Masters degree in Public Health (Social and Behavioral Sciences). She has worked in healthcare quality improvement (inpatient and outpatient), hospital and rehabilitation facility accreditation, compliance, health education and managed programs ranging from telehealth to health insurance navigation. Tierra focuses on utilizing her experiences working within health care systems to apply innovation and implementation science to bridge the gap between research and care delivery to improve the health and healthcare of ALL.

Val Slayton, MD, MPP, MBA, FACP

Dr. Slayton is the Regional Medical Director for Humana Senior Products, Medicare East Region. Before joining Humana, he held several hospital leadership positions, including Vice President Medical Affairs with Jewish Hospital and St. Mary’s Healthcare (within KentuckyOne Health) and Chief Medical Officer for UMassMemorial HealthAlliance Hospital (MA). Earlier in his career, Dr. Slayton was Chief Medical Officer for Fallon Community Health Plan (MA), Vice President Medical Affairs for HealthAmerica (PA), and Medical Director with Molina Healthcare (OH). He is currently a member of the Humana Louisville Bold Goal Board of Directors, the Louisville Health Advisory Board Behavioral Health Subcommittee, the Dare to Care Food Bank Board of Directors, and the co-chair of the Professional Training Company Support Committee at Actors Theatre of Louisville. He also served as an Examiner for the Malcolm Baldrige National Quality Award at both the national and state level.

Pat is the Director of Specialty Substance Use Programs at Bluegrass.org, and has implemented MAT with an emphasis on behavioral health centric models at the longest running substance use program in Kentucky. Bluegrass.org brings together all the treatment options into one program (which is accredited by the Joint Commission), including: individual and group therapy, psychiatric care, case management, drug testing and treatment with MAT.
