Aligning Care & Community Workshop Series: Caring for Patients with Diabetes:

We will discuss the topic of effective diabetes and pre-diabetes management with a focus on the efficacy of education and self-management in the development of diabetic treatment planning.

This event will explore: the value of the integration of interdisciplinary team approaches to diabetic healthcare delivery models, the impact of diabetes-related education in hemoglobin A1c reduction and control, the importance of adherence to national standards in diabetes-related education delivery, and localized resources for diabetes prevention.
