Needs Statement
Patients in Appalachia face unique barriers to accessing and navigating complex health care delivery systems due to distance from care and life circumstances. There is a shortage of trained patient navigators to address the needs of Appalachian patients, particularly barriers to health care, using patient navigation skills and strategies. Training is needed to provide basic definitions and orientation to the practice of patient navigation in Appalachia.Target Audience
Health Care professionals in a rural healthcare setting.Objectives
Upon completion of this activity, participants will be able to:1. Define patient navigation
2. Describe the key elements of effective patient navigation.
3. Describe key functions and skills that are important to be effective in working with Appalachian populations
