AAFP Credit
Application for CME credit has been filed with the American Academy of Family Physicians. Determination of credit is pending.

All educational sessions and meal functions will be held at The Lexington Downtown Hotel & Convention Center (formerly the Radisson Plaza Hotel), 369 W. Vine Street, Lexington, Kentucky.
Parking is available in the Central Parking garage adjacent to the Hotel and is complimentary for hotel guests. Validation stickers will be provided for non-hotel guests. Additional parking is available in the garage adjacent to the 5/3 Bank.
A block of sleeping rooms has been reserved at the group rate of $109 + tax single/double per night. Reservation deadlines are October 12, 2009, for the November session. Refund Policy
A refund less a $25 processing fee will be issued if written notice of withdrawal is received one week prior to the beginning of the activity. Registration may be transferred to another participant.
The Pharmacy and Medicine Continuing Education Office reserves the right to cancel or make substitutions to this activity. A full refund will be issued if circumstances warrant cancellation.
We reserve the right to cancel or substitute topics and/or speakers.
