Needs Statement

In order to effectively and efficiently practice Population Health Management, health care providers must be able to evaluate the processes of Population Health Management in order to make return-on-investment decisions and allocate resources properly.  

Target Audience

Practicing clinicans and healthcare learners of any field.



Upon completion of this activity, participants will be able to:

  1. Identify and measure process measures.
  2. Identify and measure cost-saving measures.
