For Survey Participants

Hello and Welcome to the Central Appalachia Inter-Professional Pain Education Collaborative (CAIPEC) webcasts!

Our CAIPEC project is a multi-faceted continuing education approach to chronic pain management targeted at health professionals in our region.

Today you will be participating in a series of module-based webcasts about chronic pain management. We will ask you to complete pre-and post-tests in order to help us understand more about chronic pain management within the practice.

With your participation in these webcasts and completion of the pre- and post-tests and evaluations, you will help us gather information about current practice ideals and knowledge gained through these webcasts.

Participation is voluntary and you may discontinue at any time. The post-test and evaluation should take approximately ten minutes to complete. Your responses to the evaluation are anonymous and all information gained will be be presented in summative form so that your responses cannot be traced back to you.

Please be aware for online evaluations, while we make every effort to safeguard your data once received on our services, given the nature of online surveys, as with anything involving the Internet, we can never guarantee the confidentiality of the data while still en route to us.

If you have questions about this project, our contact information is below. If you have complaints, suggestions, or questions about your rights as a research volunteer, contact the staff in the University of Kentucky Office of Research Integrity at (859) 257-9428 or toll-free at (866) 400-9428.

Thank you in advance for your assistance in our study!

Sincerely,
